I have exactly the same problem, I replace screen and proximity sensor started to have problems. I managed to calibrate the sensor but the problem persisted.

Go to settings - display - proximity sensor calibration.
Also maybe you have some alien object in sensor as dust etc.

If i try to calibrate the sensor it gives me a fail. I don't have any dust or object in the sensor.

you replace the screen? I have noticed that the people who have replaced screen have this problem . when try to calibrate gives fail. I have the same problem and using 7.0. but recently I was testing with the engineer mode and noticed that the sensor worked perfectly. I calibrated it and still the problem persisted.
