Umi Sim Card Slot Broken and Screen is Coming off.

I am expeienceing stouble with my Umi Plus. I have had the phone aproximatly 4 months. A message in the phone keeps poping up that the sim card has poped out. The screen of the phone just started to come off with out any falls
