Please check for updates as latest version was fixing proximity sensor issues.
Unfortunately sometimes you need perform system flash as error is existing in main partition.
To do this please use SPFT and follow this instruction

Also please check for outside sources as maybe you have applied screen protector which interference to proximity sensor.
In settings - display you have proximity and G sensor calibration options
